The PS5-exclusive launches September 15, 2026, and the rating makes clear this isn't the Marvel experience parents might expect from the studio behind one of the most kid-friendly superhero franchises in gaming.
What the rating actually covers
The ESRB lists Blood and Gore, Drug Reference, Intense Violence, Partial Nudity, and Strong Language as content descriptors. In practice, that means claw attacks that result in dismemberment and decapitation, slow-motion finishing moves — including driving claws through an enemy's skull — and locations littered with severed limbs. Cutscenes push further, showing characters torn apart or beheaded outright. One character's bare backside makes a brief appearance, and dialogue includes references to controlled substances alongside frequent profanity. This is Insomniac's most mature game to date; for comparison, both Spider-Man titles carried a T for Teen rating.
The gore toggle
Not every player wants a full helping of ultra-violence, and Insomniac has built in an option to turn off blood, dismemberment, and related content through the settings menu. Game director Mike Daly has been upfront about it, saying the extreme violence is "not for everyone." That's a notable accessibility move for an AAA action game leaning so hard into its mature content — and a practical answer to parents buying for younger teens who are fans of the character from films.
